Services are provided and billed by Greg Crumley, dba Greg Crumley LLC. I participate in some of the major health insurance plans in the region. Following are the health plans which we currently accept.
If your plan is not listed, we will be considered to be "out-of-network" and will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to assist in getting reimbursements sent directly to you. We will make every effort to be included in any insurance plan you might be participating in.
Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.
For those who do not have coverage under insurance we offer an "ability-to-pay" scale that is based on income.

What if my insurance company is not on the list of health plans?
We will work with your insurance to become part of their network. Until that is completed we will be considered to be "out-of-network" for insurance purposes and the policies of your insurance policy for "out-of-network" providers will apply.
What billing or insurance information will I receive?
​We can provide you with a statement of your account when requested.

How long will it take to get things settled with the insurance company?
Typically it can it take up to 30 days to obtain payment from an insurance carrier. Please remember that the client is responsibile for any co-payments and deductibles as set fort in the policy.​

​I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?
​If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
